Record the decision and its assumptions

Start with the choice you need to make, the customer problem and the assumptions supporting the proposed direction. Keep the record short enough to maintain. Its purpose is to help the team remember why it believed the idea was worth pursuing.

Distinguish observation from interpretation

Record where evidence came from, what was observed and how the team interpreted it. Note important limitations, such as a small or narrow group of participants. This makes it easier to compare new findings with earlier reasoning without treating every note as equally conclusive.

Review what changes

When new evidence arrives, ask which assumption it supports or challenges. Update the decision and the next learning step. A useful log shows how understanding evolved; it should not become an archive of documents nobody consults.
